We need your help! The New York Organ Donor Network is asking YOU to help raise awareness for organ donation. Grab a camera and create a commercial or short video clip that encourages New Yorkers to sign up to be organ donors. Would your commercial be funny? Moving? Shocking?

Contest Instructions

Every video submitted helps to spread the importance of organ donation and spark conversation. Just ONE organ donor can save up to 8 lives! Videos must be no longer than 2 minutes in length, address the topic of organ donation and include a call to action. The New York Organ Donor Network, joined by industry executives in advertising, film and public advocacy, will judge entries and post finalists for a public vote on November 18, 2011. NO PURCHASE NECESSARY TO ENTER OR WIN
New York Organ Donor Network “Think You’re Hollywood?” Video Contest (“Contest”) starts on Monday, October 3, 2011 at 12:01 a.m. Eastern Time (“ET”) and ends on Friday, November 11, 2011 at 11:59 p.m. ET (“Submission Period”). 
Qualified judging of videos and selection of five (5) finalists will begin on Saturday, November 12, 2011 and end on Friday, November 18, 2011 (“Judging Period”).  
Public voting for one (1) final winner starts on Saturday, November 19, 2011 at 12:01 a.m. ET and ends on Friday, December 2, 2011 at 11:59 p.m. ET (“Voting Period”). 
ELIGIBILITY: Contest is open to legal residents of the fifty (50) United States and the District of Columbia who are 18 years of age or older.  Employees of the New York Organ Donor Network (“Sponsor”), and its respective parents, subsidiaries, affiliates, promotion and advertising agencies, and members of their immediate family (spouse and parent, children and siblings and their respective spouses, regardless of where they reside) and persons living in the same household, whether or not related, of such employees, are not eligible to enter or win.  Void where prohibited by law. Void in Puerto Rico, US territories and possessions, and where prohibited or otherwise restricted by law.  Contest is subject to all applicable federal, state and local laws.

CONTEST BRIEF OVERVIEW: The New York Organ Donor Network is asking YOU to help us raise awareness for organ donation. Create your very own commercial or short video that moves New Yorkers to sign up to be organ donors.  Would your video be funny? Moving? Shocking? Put your creative skills into action. Simply grab your video camera and upload a short video or commercial moving New Yorkers to sign up to be organ donors. How would you inspire change and help save lives? Be a part of the movement and ignite conversation! 
HOW TO ENTER: To enter the Contest, you (“Entrant”) shall, during the Submission Period, (1) accept the Sponsor’s Facebook application called “Think You’re Hollywood?” (“Application”), available at http://www.facebook.com/NewYorkOrganDonorNetwork (“Sponsor’s Facebook Page”); (2) follow the directions provided to complete the entry form; and (3) upload an original video (each a “Video”) which you intend to enter into the Contest (completion of all steps collectively referred to herein as an “Entry”). If you do not already “like” the Sponsor’s Facebook Page, you will be required to do so in order to access the Application (you can “unlike” the Sponsor’s Facebook Page at any time after entry).
ENTRY REQUIREMENTS: Video must be submitted in .avi, .mov, .flv, .mpg, .mpeg, .wmv, or .mp4 format must be no longer than two (2) minutes in length and cannot exceed 1024 MB in size. Entrant shall not use any music in the Video unless such music is an original and unpublished composition of the Entrant.  Any individuals shown in the video must be at least 18 years of age (unless Entrant is the parent or legal ward of such individual). In the event that the winning Entry includes performances by people other than the Entrant, only the Entrant who submitted the Entry shall be deemed the winner and only the Entrant shall receive the Prize. Entries generated by script, macro or other automated means, or by any means which subvert the entry process, will be deemed null and void and Entrant of such Entry will be disqualified. Entrant may submit more than one Entry, provided each Entry is substantially different from the other(s) Entry(ies) submitted, as determined by the Sponsor. In the event of a dispute as to any Entry, the authorized account holder of the email address used to register will be deemed to be the Entrant and he/she must comply with these Official Rules. Entries received from any person/email address in excess of the limitation will be void.  All Entries become the sole property of the Sponsor and will not be returned.
CONTENT RESTRICTIONS: Entry may not contain, as determined by the Sponsor, in its sole discretion, any content that:is sexually explicit or suggestive; unnecessarily and/or excessively violent, gruesome or derogatory of any ethnic, racial, gender, religious, professional or age group; profane or pornographic; contains nudity;promotes alcohol, illegal drugs, tobacco, firearms/weapons (or the use of any of the foregoing); promotes any activities that may appear unsafe or dangerous; promotes any particular political agenda or message;is obscene or offensive, or that endorses any form of hate or hate group; appears to duplicate any other submitted Entries;  defames, misrepresents or contains disparaging remarks about Sponsor or its products or services, other people or companies;contains trademarks, logos, or trade dress (such as distinctive packaging or building exteriors/interiors) owned by others, without permission (to the extent permission is required); contains any personal identification, such as license plate numbers, personal names, e-mail addresses or street addresses;contains copyrighted materials owned by others (including music, photographs, sculptures, paintings, and other works of art or images published on or in websites, television, movies or other media), without permission,  to the extent permission is necessary;contains materials embodying the names, likenesses, voices, or other indicia identifying any person, including, without limitation, celebrities and/or other public or private figures, living or dead, without permission, to the extent permission is necessary; contains look-alikes of celebrities or other public or private figures, living or dead;communicates messages or images inconsistent with the positive images and/or good will with which Sponsor wishes to associate; and/orviolates any law.
Sponsor reserves the right to not post any Entries that contain any of the prohibited content above or for any other reason whatever. Entry must be the original work of the Entrant, may not have been previously published, may not have won previous awards, and must not infringe upon the copyrights, trademarks, rights of privacy, publicity or other intellectual property or other rights of any person or entity.  If the Entry contains any material or elements that are not owned by the Entrant, and/or which are subject to the rights of third parties (e.g., if someone other than Entrant filmed the video), the Entrant is responsible for obtaining, prior to submission of the Entry, any and all releases and consents necessary to permit the use and exhibition of the Entry by Sponsor in the manner described in these Official Rules, including, without limitation, name and likeness permissions from any person who appears in or is identifiable in the Entry. Sponsor reserves the right to request from Entrant proof of these permissions, as well as proof of parental/legal guardianship status over any individuals under 18 who appear in the Entry, in a form acceptable to Sponsor at any time. Failure to provide such proof may, if requested, render Entry null and void and Entrant may be disqualified.  By submitting an Entry, Entrant warrants and represents that (i) his/her Entry complies with these Official Rules and (ii) he/she and any persons appearing or who are identifiable in the Entry consent to the submission and use of the Entry in the Contest and to its use by Sponsor as otherwise set forth herein.
By submitting an Entry, Entrant hereby grants permission for the Entry to be posted on Sponsor’s websites and blogs, including Sponsor’s social media sites (e.g., Facebook, Twitter, YouTube, etc.) and otherwise be used in accordance with the grant of rights set forth herein. Entrant agrees that the Released Parties (as defined below) are not responsible for any unauthorized use of Entries by third parties. 
WINNER DETERMINATION:
Judging Phase: All eligible Entries received during the Submission Period by Sponsor will be judged to select up to five (5) finalists (collectively, the “Finalists”) during the Judging Period by a panel of qualified judges based equally on the following criteria: (1) originality, (2) overall impact, and (3) addressing the overall theme (i.e., raising awareness for organ donation) (the “Judging Criteria”).  In the event of a tie, an additional, “tie-breaking” judge will determine the finalist(s) based on the criteria listed herein. Sponsor reserves the right to advance fewer than five (5) finalists to the voting phase if, in its sole discretion, it does not receive a sufficient number of eligible and qualified Entries. 
Voting Phase: Finalists’ Entries will be featured at http://www.Facebook.com/NewYorkOrganDonorNetwork during the Voting Period.  Visitors to the page will be able to select the Finalist’s Entry that they believe is the most impactful video in creating awareness and encouraging organ donorship. Visitors may vote for up to three (3) Entries per day during the Voting Period; however, voting is limited to one (1) vote per person/per email address per Entry per day. Votes received from any person or email address in excess of the stated limitation will be void. Votes obtained by any fraudulent or inappropriate means, including, without limitation, trading votes or offering prizes or other inducements to members of the public, payment for votes or offering to trade votes, as determined by Sponsor in its sole discretion, may result in those votes being void and the associated Finalist being disqualified. The Prize will be awarded to the Finalist who receives the highest number of valid votes, as determined by Sponsor in its sole discretion. In the event of a tie, a “tie-breaking” judge will determine the winner based on the Judging Criteria.  Use of script, macro or any automated system to vote or with the intent to impair the integrity of the voting process is prohibited and all such votes will be void.  Sponsor’s decisions are final and binding on all matters relating to this Contest. The Prize award is subject to verification of eligibility and compliance with these Official Rules.
WINNER NOTIFICATION: The potential winner will be notified on or about December 5, 2011 by telephone and/or email and may be required to complete an Affidavit of Eligibility, Liability and Publicity Release (unless prohibited by law) and any other documents required by Sponsor, which must be returned within three (3) days of the date appearing on prize notification. Return of prize or prize notification as undeliverable, failure to sign and return requested documentation within the specified time period, the inability of Sponsor to contact a potential winner within a reasonable time period or noncompliance with these Official Rules by any potential winner will result in disqualification and, at Sponsor’s sole discretion, the Prize may be awarded to a runner-up.  If any potential winner is at least 18 but still considered a minor in his/her jurisdiction of residence, Sponsor reserves the right to award the prize in the name of his/her parent or legal guardian who will be responsible for fulfilling all requirements imposed on winner set forth herein. 
Prize (1): A trip for winner and one guest to Pasadena, California. Trip package includes round-trip, coach-class air transportation for two (2) from a major airport near winner’s home (determined by Sponsor in its sole discretion) to an airport near destination; five (5) nights' accommodations at Sheraton Pasadena Hotel one (1) room, double occupancy); two (2) tickets to the Rose Bowl football game (the “Game”); two (2) guest passes to the Tournament of Roses Parade (the “Parade”); and ground transportation to and from Rose Bowl, Rose Bowl Parade and hotel accommodations. ARV: $5,000. Winner must complete the trip on or between the following dates: December 29, 2011 and January 3, 2012 or prize will be forfeited and Sponsor will have no further obligation to winner. Trip must be booked at least fourteen (14) days prior to departure.  Trip subject to availability and blackout dates. Travel must be round-trip. Sponsor will determine airline and flight itinerary in its sole discretion. No refund or compensation will be made in the event of the cancellation or delay of the Game, Parade or any flight. Travel and accommodations are subject to availability. Travel is subject to the terms and conditions set forth in this Contest, and those set forth by the Sponsor’s airline carrier of choice as detailed in the passenger ticket contract. All expenses and incidental travel costs not expressly stated in the package description above as being provided, including but not limited to, daily ground transportation, meals, incidentals, passenger tariffs or duties, airline fees, luggage fees, surcharges, airport fees, service charges or facility charges, personal charges at lodging, security fees, taxes or other expenses are the responsibility solely of winner.  Unless child or legal ward of winner, travel companion must be at least eighteen (18) years of age, or the age of majority in his/her jurisdiction of residence, whichever is greater, as of the date of departure or a sibling/friend of winner with the appropriate parental permissions and releases and must travel together on same itinerary and at the same time as the winner. Travel companion (or his/her parent or legal guardian) must execute liability/publicity releases prior to issuance of travel documents. Winner and travel companion are solely responsible for necessary documents. Travel restrictions, conditions and limitations may apply. If in the judgment of Sponsor air travel is not required due to winner’s proximity to prize location (within 100 miles), ground transportation will be substituted for round-trip air travel at Sponsor’s sole discretion and no compensation or substitution will be provided for difference in prize value. Sponsor will not replace any lost, mutilated, or stolen tickets, travel vouchers.  Actual value may vary based on airfare fluctuations and distance between departure and destination and dates of travel. Winner will not receive difference between actual and ARV. 
USE OF ENTRIES:  By submitting an Entry, each Entrant agrees Sponsor shall have a nonexclusive, unlimited, perpetual, worldwide, royalty-free, fully paid-up license to the Entry submitted (including all rights embodied therein) and that it and Sponsor and its designees may exploit, copy, recreate, edit, modify, post, and distribute the Entry and all elements of such Entry, including, without limitation, the names and likenesses of any persons or locations embodied therein, in any and all media now known or hereafter devised, without compensation, permission or notification to Entrant or any third party for the purpose of marketing and/or promoting Sponsor, and its mission and services (including, without limitation, to use the Video or recreate Sponsor’s own version of the Video in connection with an advertising campaign). Sponsor is under no obligation to make use of any of the rights granted herein.
GENERAL CONDITIONS:  By participating, each Entrant agrees: (a) to abide by these Official Rules and decisions of Sponsor, which shall be final and binding in all respects relating to this Contest; (b) to release, discharge and hold harmless Sponsor, and its respective parents, affiliates, subsidiaries, and advertising and promotion agencies, and the respective officers, directors, shareholders, employees, agents and representatives of the forgoing (collectively, “Released Parties”) from any and all injuries, liability, losses and damages of any kind to persons, including death, or property resulting, in whole or in part, directly or indirectly, from Entrant’s participation in the Contest or any Contest-related activity or the acceptance, possession, use or misuse of any awarded prize; and (c) to the use by Sponsor and its designees of his/her name, city and state of residence, voice, performance, video/photograph, image and/or likeness for programming, advertising, publicity and promotional purposes in any and all media, now or hereafter known, worldwide in perpetuity, without compensation (unless prohibited by law) or additional consents from  Entrant or any third party and without prior notice, approval or inspection, and to execute specific consent to such use if asked to do so.  
MISCELLANEOUS: The Released Parties are not responsible for lost, late, incomplete, damaged, inaccurate, stolen, delayed, misdirected, undelivered, or garbled Entries or votes; or for lost, interrupted or unavailable network, server, Internet Service Provider (ISP), website, or other connections, availability or accessibility or miscommunications or failed computer, satellite, telephone or cable transmissions, lines, or technical failure or jumbled, scrambled, delayed, or misdirected transmissions or computer hardware or software malfunctions, failures or difficulties, or other errors or difficulties of any kind whether human, mechanical, electronic, computer, network, typographical, printing or otherwise relating to or in connection with the Contest, including, without limitation, errors or difficulties which may occur in connection with the administration of the Contest, the processing of Entries,  the tabulation of votes, the announcement of the prize, the incorrect uploading of the photo or in any Contest-related materials.  Released Parties are also not responsible for any incorrect or inaccurate information, whether caused by site users, tampering, hacking, or by any equipment or programming associated with or utilized in the Contest.  Released Parties are not responsible for injury or damage to any person's computer related to or resulting from participating in this Contest or downloading materials from or use of the website.  Persons who tamper with or abuse any aspect of the Contest or website, who act in an unsportsmanlike or disruptive manner or who are in violation of these Official Rules, as solely determined by Sponsor, will be disqualified and all associated Entries/votes will be void. Should any portion of the Contest be, in Sponsor’s sole opinion, compromised by virus, worms, bugs, non-authorized human intervention or other causes which, in the sole opinions of the Sponsor, corrupt or impair the administration, security, fairness or proper play, or submission of Entries, Sponsor reserves the right at its sole discretion to suspend, modify or terminate the Contest and, if terminated, at its discretion, select the potential winners from all eligible, non-suspect Entries received prior to action taken using the winner determination procedure outlined above. CAUTION: ANY ATTEMPT TO DELIBERATELY DAMAGE THE SPONSOR WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE CONTEST MAY BE IN VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVE THE RIGHT TO SEEK DAMAGES AND OTHER REMEDIES (INCLUDING ATTORNEYS’ FEES) FROM ANY SUCH INDIVIDUAL TO THE FULLEST EXTENT OF THE LAW, INCLUDING CRIMINAL PROSECUTION.
