Conditions of Submission of Personal Stories to the New York Organ Donor Network:
By submitting personal stories to the Donor Network, individuals agree to the following statement:
I agree that by submitting this personal story, the New York Organ Donor Network will review it and decide whether to use it to help publicize transplantation in general and to promote the need for organ, eye and tissue donors. The New York Organ Donor Network will notify me of their decision. The New York Organ Donor Network will not use this story without my written permission.
If the story is used, and any photographs that I should subsequently submit, the Donor Network will contact me to obtain further information, and allow me the opportunity to discuss how the story may be used: For example, on the New York Organ Donor Network website, offered to the media, etc. In the event that my story and photos are used by the Donor Network, I will give my written permission by way of a Media Release Consent Form.
