Q: Why do we need an Organ and Tissue Donor Registry?
A: In New York State, family consent is requested for organ and tissue donation. Many families do not consent to donation because they are unsure of their loved ones' wishes. The New York State Organ and Tissue Donor Registry ensures that a person's wishes to become a donor upon his or her death will be honored by family members and health care providers. By enrolling in the registry, a person shows their intent to become a donor.


Q: How do I enroll in the New York State Organ and Tissue Donor Registry?
A: You will be enrolled automatically if you check the organ donor box on your driver's license or non-driver identification (ID) card application or renewal form. You can request that a registry enrollment form be mailed to you, by calling 1-800-GIFT-4-NY (1-800-443-8469) Or, you can enroll online at http://www.health.state.ny.us/nysdoh/donor/index.htm or by clicking here on this website.


Q: What happens when I say "yes" to enrollment?
A: Enrollment in the New York State Organ and Tissue Donor Registry declares your intent to donate your organs and tissues after your death. When this occurs, the New York Organ Donor Network (the Greater New York metropolitan area's organ procurement organization) or tissue bank will inform your family of your decision. At this juncture, your family will still need to give consent, so it is crucial that you tell them about your wishes beforehand.
Q: If I enroll in the New York State Organ and Tissue Donor Registry, should I still sign the back of my driver's license?
A: Yes, you should still sign the back of your driver's license or non-driver ID card. This will help to ensure that your wishes will be carried out.

Q: Can I change my mind after I enroll?
A: Yes. If you decide you do not wish to become an organ or tissue donor and you want your name removed from the registry, please write to:
