| Once you have enrolled in the New York State Organ and Tissue Donor Registry (and, preferably, also signed your driver's license), be sure to discuss your decision with your closest family members. |

| In the event that one day you are considered to be medically suitable as an organ and/or tissue donor, your family would be given the opportunity to consent to donate. If they know what your wishes are in advance, they are more likely to say "yes" when asked to make a decision about donation. |

| To help make your decision about donation known to your next of kin, print out the Family Notification Form in this section, fill it out, and give the form to your loved ones. They will then know, in writing, that you have made an informed decision about donation. They will understand that you wish to donate life. |
